Description:
Assisted Living provides long-term housing and care for seniors with a sense of community in a safe residential setting. Residents are generally active but may need support with activities of daily living such as, bathing, dressing, and using the toilet. Seniors in assisted living can expect personalized care, nutritious meals, a wide range of social activities to cater to a variety of interests.

Memory care, also called Alzheimer's care, provides housing and 24-hour care for seniors with various forms of dementia. These facilities offer a safe environment for residents focused on improving quality of life, reducing confusion, and preventing wandering. These communities are residential spaces that cater to the specific needs of seniors with memory loss. The key services of this type of senior care include 24-hour assistance, specialized staff, memory-enhancing therapies, a secure environment and more.

Nursing Homes provide nursing care and long-term housing for seniors. Elderly adults who need long-term care often have significant physical or mental health conditions that require 24-hour care. These facilities offer the highest level of care for seniors who need medical care yet, do not require hospitalization. Nursing homes also provide short-term, rehabilitative care after a hospital discharge. The rehabilitation programs help seniors, recovering from an illness or accident, to regain mobility and strength in order to return to normal activities.

Independent Living offers a carefree lifestyle for seniors who want to enjoy retirement with ease. These facilities provide variety of convenient services and amenities. Some include prepared meals, transportation, social activities, and housekeeping. Unlike assisted living, independent senior living does not provide personalized care (bathing, toileting, etc.) In fact, seniors at independent living facilities are generally active, healthy, and able to live on their own but may desire convenience and more social opportunities.

Senior Apartment communities are tailored to the needs of active adults over the age of 55. Senior communities often feature amenities such as golf swimming pools, tennis courts, and fitness centers to attract the older adults who want a low maintenance lifestyle. Most community fees include utilities, landscaping services, home maintenance services and security. Senior apartments also do not offer the type of personal care services that other facilities may provide.

Residential care homes offer a smaller, more home-like family setting for seniors. Care homes include food services and assistance with daily living but do not typically have a medical professional on-site. Part-time medical care is offered, but is not a primary focus. These types of residences are typically located in traditional homes and neighborhoods that provide care for fewer patients than assisted living communities. It is uncommon for a care home to offer services for more than 10 seniors; which helps create the more personal, home-style living environment.

Home Care is a service that provides senior care within an individual’s residence. At-home care best serves seniors who want to age in place while desiring companionship or require support with daily chores and activities of daily living (dressing and bathing). The services that an at-home caregiver provides will depend on a senior's individualized needs. While some may rely on home care providers for help with errands, cooking, and housekeeping, others need intensive care in their home to manage incontinence, mobility issues, or dementia. In-home care may increase peace of mind for family members and enhance senior safety, resulting in one-on-one bond between a senior and their caregiver.

Supportive Housing Program

Our Supportive Housing Program provides housing and case management services for people who are homeless and unable to afford housing because of a physical or mental disability. We currently have 4 one-bedroom apartments for single adults. Our case managers work with tenants on an on-going basis to help them remain in stable housing and meet their health care needs. Funding is provided through a grant from the U.S. Department of Housing and Urban Development (HUD), program fees paid by tenants, private donations, and fundraising activities. This program provides permanent supportive housing to individuals or families who have become homeless as a result of a physical or mental disability. It consists of six scattered site apartments which are leased to Sierra HOPE.

Our program participants include people with physical disabilities and people with severe mental health disorders. They may remain in the Permanent Supportive Housing program as long as necessary. Participants are charged a program fee of up to 30% of their monthly income to help cover the cost of services provided to them. Those who have no income at program entry receive assistance to apply for benefits they may be eligible for, including Social Security Disability Insurance, which can be a lengthy process. Our case management team works closely with each resident to assess their needs and develop a personalized care plan, and provide on-going monitoring and advocacy to help them access needed services, maintain compliance with medications and treatment plans, and accomplish individual goals towards improved self-sufficiency.

Sierra HOPE works closely with other service providers including non-profit organizations, local government agencies, medical providers, and mental health services to obtain appropriate referrals and coordinate care.
