A place for mom Added on January 10, 2023

  • Category: Housing
  • Counties: Amador, Calaveras, Mariposa, Tuolumne, San Joaquin, Stanislaus

Description:
Assisted Living provides long-term housing and care for seniors. Assisted living residents are generally active but may need support with activites of daily living such as bathing, dressing, and using the toilet. Seniros in assisted living can expect personalized care, nutritious meals, a wide range of social activites to cater ti a variety of interests, and a sense of community in a safe residential setting. Memory Care, sometimes called Alzheimer's care, providese housing and 24-hour care for seniors with alzheimers's disease and other forms of dementia. Memory care facilities offer a safe, uniquely designed environment for residents with a focus on improving quality of life, reduing confusin and preventing wandering. But what exactly is a memory care facility? Memory care communities are residentail spaces that cater to the specific needs of seniros with memory loss. Key servies and features that make memory care facilities different from other types of senior care include 24-hour care, specialized staff, memory-enchancing therapies, a secure environment and more. Nursing Homes, Somtimes called skilled nursing facilites, provdie nursing care and long-term housing for seniors. Elderly adults whp need long-term care at a nursing home facility often have significant physical or mental health conditions that require 24-hour nursing and personal care. These facilites offer the highest level of care for seniors who need medical care, but don't require hospitalization. NUrsing homes also provide short-term, rehabilitative care after a hospital discharge. Rehabilitation programs at nursing home facilities helpp seniors recovering from an illness or accident regain mobility, Strength and function until they can return to normal activites. Independent Living offers a carefree lifestyle for older adutls who want to enjoy retirement with ease. Seniro independent living facilites providea variety of convenient services and amenities, including prepared meals, transportation social activities and housekeeping. Unlike assisted living , independent seniros living doesn't provide help with personal care like bathing or toileting in fact seniors at independent living facilities are generally active, healthy and able to live on their own but may desire convenience and more social opportunites. Senior Apartment communities are tailored to the needs of active adults over age 55. Senior 55p lus communites ofeten feature amenities such as golf swimming pools, tennis courts, and fitness centers to attract the older adults who want a low maintenance lifestyle. Most community fees inclsude utilities , landscaping services home maintenance services and security. Senior apartments do not offer the type of personal care services that assisted living communites and memory care facilities provide Residential care homes offer a smaller, more home-like family setting for seniors. Care homes offer food services and assistance with daily living but do not typically have a medical professional on-site. Limited , part-time medical care is offered, but it's not a primary focue of this type of senior living community. Care home residences are typically located in traditional homes and neighborhoods and provide care for fewer patients than assisted living communities. It us uncommon for a care home to offer services for more than 10 seniors; which helps create the more personal, home-style living environment. Home Care is a service that provides senior care within an individual;s residence. Care at home best serves seniors who want to age in place, but who may desire companionship or rewuire support with daily chores and activities of daily living such as dressing and bathing. The services that an at-home caregiver provides will depend on a senior's individualized needs. While some seniors may rely on home care providers for help with errands, cooking, and housekeeping others need intensice care in ther home to manage incontinence, mobility issues, or dementia. In-home care may increase peace of mind for family members, enhance senior safety, and result in a deeper, one-on-one bond between a senior and their caregiver.

Over 80% of the guest families we serve have obtained stable housing. Once a family finds a place to live, we continue to help them transition to a self-sufficient lifestyle. Families arenÕt alone on their journey. We work together to set goals, celebrate progress and provide support every step of the way. Housing Stabilization How does Family Promise help families on their journey to stable housing? All adults sheltering with Family Promise agree to participate in our intensive case management services. Families set up a plan to find work and manage their resources Ð always with a goal toward providing a secure home for their children. The We Care Program (WCP) provides shelter services for 49 homeless men a night. Through our hygiene program, we provide individual adults and families experiencing homelessness in encampments or on the street, a place to do laundry and take a shower. The center is open Monday through Friday from 12:00 pm to 3:00 pm. We operate on a first-come, first-served basis.
